COCSD Athletics packet and concussion doc Go to the "Athletics" tap on the left of this website to download the COCSD Athletics packet and the new Concussion information packet. You will find the forms in PDF format under the "Teams" section, on the "sports packet information" link. Students and parents MUST complete BOTH packets prior to the start of a new sport. Oak Creek School will be using this packet this year for ALL athletics offered, and students and parents only need to fill this information out ONE TIME for students to participate in ANY sport. Parents may download both documents to their computer (you need Adobe as they are PDF format), or they can stop by the OCS office to pick up a copy. |
